Swanton House Care Centre comprises of three different accommodation areas supporting people who have a diversity of need. All three components of Swanton House Care Centre provide 24 hours care and nursing support for people who have a range of complex needs associated with their mental health or learning difficulty. The service consists of: Holly Court and Bluebell, both of which are purpose built single story buildings. Also on the site is the house which is an historic Grade II listed building all set in extensive and attractive formal and informal grounds.
Swanton House Care Centre provides the environment that support adults with mental health and learning difficulties. The support team work closely with the individual, their family, the local community and associated professionals to develop a structured and meaningful person-centred approach to care and support. The support team actively encourages people to make choices and take control within their capacity, through individually designed and personalised support planning. Direct decision making and personal development with a real emphasis on individual outcomes is a focus of the support team.
The Registered Manager, Dot Medland, has over 30 years’ experience in social care and has successfully managed homes within elderly care, mental health and learning disabilities services, promoting the rights of the people who use the services and championing person-centred support. Our team of highly-qualified and attentive staff focused on the individual needs of the people who live at Swanton with a tailored care plan for each person. The support team, which is a balanced mix of social carers and specialist nursing, aim to promote self-esteem and confidence by developing personal, social and life skills to enhance individual development and positively manage associated health and emotional needs.
